Prevention, Education and Outreach Coordinator (Nutrition)

The Prevention, Education and Outreach Coordinator facilitates, coordinates, implements, develops and evaluates prevention and community based outreach programs at Children's Hospital Colorado (CHCO).  The Coordinator provides moderate to complex programming coordination, including coordinating with the relevant community partners. The Coordinator may be required to provide training to external and internal partners and assist with the development of programming designs.

Specifically, the Prevention Education and Outreach Coordinator will have three significant areas of influence.  The first will be advising food procurement practices adhering to sound nutrition science for the Healthy Roots Food Clinic.  Second, the PEOC will develop condition specific nutrition education sequences for visitors to the Healthy Roots Food Clinic.  These education sequences will follow the established visit protocol.  Lastly, the PEOC will participate in the daily operations of the Healthy Roots Food Clinic.

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

  • Coordinates moderate to complex program specific outreach and events and maintains program branding materials including brochures and websites.
  • Serves as liaison between program, department, CHCO and community partners.
  • Supports development and implementation of population health programs based on community benefit strategic plan, including activities designed to improve health, safety and family education/empowerment.
  • Assists with the development of prevention, education, and outreach programming; and delivers and provides training/outreach to internal and external partners.
  • Participates in local, state, regional and national efforts related to issues identified by the CHCO community needs assessment.
  • Contribution to keep education sequences nutritionally sound and congruent with current research
  • Applies nutritional standards to all practices in the Healthy Roots Brand 

SCOPE AND LEVEL

Guidelines: Guidelines are generally numerous, well established, and directly applicable to the work assignment.  Work assignment and desired results are explained by general oral or written instructions.

Complexity: Duties assigned are generally repetitive and restricted in scope but may be of substantial intricacy.  Employee primarily applies standardized practices.

Decision Making: Decisions or recommendations on non-standardized situations are limited to relating organizational policies to specific cases.  Brings non routine issues to supervisor. Problems that are not covered by guidelines or are without precedent are taken up with the supervisor.

Communications: Contacts with the public or employees where explanatory or interpretive information is exchanged, gathered, or presented and some degree of discretion and judgment are required within the parameters of the job function.

Supervision Received: Under normal supervision, within a standardized work situation, the employee performs duties common to the line of work without close supervision or detailed instruction.  Work product is subject to continual review. 

 

MINIMUM QUALIFICATIONS 

EDUCATION – High School Diploma.

EXPERIENCE – Four years of experience coordinating and implementing prevention and education initiatives that help address social determinants of health.

EQUIVALENCY – A degree may substitute for work experience on a year for year basis.


Salary Information
Pay is dependent on applicant's relevant experience.

Annual Salary Range (Based on 40 hours worked per week): $61,473.39 to $92,210.08
