Case Management at Children's Hospital Colorado

Case managers are experienced registered nurses who work closely with patients, families and other members of the medical team to coordinate each patient's discharge plan. A case manager will advocate for your family during every step of your child's care.

Discharge planners are non-licensed staff members who collaborate with our case managers and assist in scheduling appointments and arranging medical equipment to use at home.

How does a case manager help your family?

While other members of the care team treat your child's physical needs, our team offers services to support your child for their safe transition home.

While your child is in the hospital

Case managers organize the plan of care from your entire care team to help your child get home safely and with everything they need to succeed after leaving the hospital.

Case managers will offer suggestions to the medical team regarding discharge. They can also help your family coordinate meetings with your child's medical team.

When it’s time for your child to leave the hospital, your case manager will review the discharge plan with your family, updating the plan to meet your child’s needs.

Our team ensures your child’s primary care provider (PCP) has the information they need about your hospital stay to support your family after your child returns home.

The Discharge Plan

Your child’s discharge plan will ensure your family is supported when you leave the hospital. The plan will make sure that your child has the services that they need, such as:

  • Disposable medical supplies
  • Family caregiver programs (CNA/IHSS)
  • Home nursing
  • Mobility and safety equipment
  • Occupational, physical and speech therapy
  • Specialized IV medicine for home
  • Tube feeding supplies

If needed, case managers will help you find a homecare company that meets your child's needs and is covered by your health insurance. We can also help you connect with additional care coordination resources after your child leaves the hospital, if needed.

Coordinating care with your health insurance

Case managers can help you find the right people to answer questions about health insurance and paying for care, including authorizations and general billing questions. They can connect you with financial assistance and counseling.
